Advance Care Planning at the Library

Community members are invited to the free “Advance Care Planning” program at the Grants Pass and Illinois Valley branch of Josephine Community Library.

Join Angela Franklin, Adult Behavioral Health Specialist and Suicide Prevention Coordinator for Options for Southern Oregon, to explore advance care directives, Physician Orders for Life Sustaining Treatment (POLSTs), after-death care directives, and conversations with loved ones. Participants will learn ways to legally appoint healthcare decision-makers and final disposition agents and leave with the knowledge to ensure end-of-life and after-death wishes are honored.

This program is offered at the Grants Pass and Illinois Valley library branches on the following days:

This program is offered at no charge and open to the public. Registration is not required, and a library card is not necessary to participate.

Angela Franklin is an Older Adult Behavioral Health Specialist and Death Midwife. She has spent many years working to support communities in creating and maintaining death sovereignty.
